We are looking for a Complex Care Clinical Lead (RGN/RSCN) to join a well-established and progressive provider of complex care packages within the community. This community role will be based in the North Londonarea where the company has secured further packages during an exciting growth phase.

The successful Complex Care Clinical Lead will be responsible for providing face to face support to children and their families, whilst creating personalised care plans for each child. You will work closely with a core team of carers; training and supporting them with their clinical skills and activities.

Criteria For a Complex Care Clinical Lead

  • RGN or RSCN with a minimum of 12 months community complex care experience and an active NMC PIN
  • Supportive management style that can bring out the best in others
  • Up to date experience of tracheostomy care and ventilators within the last 2 years
  • Advanced communication skills verbally and written to liaise effectively with patients, families and the MDT
  • Suitable for Band 5 / Band 6 Nurses with evidence of effective leadership and management of HCAs
  • Full UK Driving License is essential

Key Responsibilities For a Complex Care Clinical Lead

  • Design and deliver a tailored care plan specific to an individual clients needs
  • Effectively manage the MDT through strong communication and teamwork
  • To provide ongoing monitoring, support and assessment of clients physical and physiological needs within your caseload
  • To deliver nursing interventions including the management of medicine delivery
  • Build and maintain sophisticated relationships with your clients family
  • Working closely with community teams, CHC nurses and CCG commissioners to establish and maintain bespoke packages of care

Benefit Package For a Complex Care Clinical Lead

  • Up to £55,000 per annum DOE
  • 2 days working from home per week / 3 days in the community
  • Monday to Friday (9-5) hours
  • 33 days holiday
  • Private pension scheme
  • Business travel expenses of £0.45 per mile

  • Company Mobile Phone and Laptop
