Job summary

Elevation Recruitment Group are recruiting on behalf of a leading specialist care organisation for a newly created Complex Assessments Manager position. This is a hybrid role covering Lancashire and Yorkshire, offering 37.5 hours per week and a genuine strategic influence over service development and patient outcomes.

You will lead the assessment, progression and mobilisation of complex referrals across Learning Disabilities, Autism, Mental Health and Complex Care, working closely with NHS

Qualifications

  • 3+ years in a senior practitioner, assessment or commissioning role.
  • Experience with Learning Disabilities, Autism, Mental Health and Complex Care.
  • Strong assessment, risk management and report-writing skills.
  • Knowledge of safeguarding, restrictive practices and placement coordination.

Responsibilities

  • Manage complex referrals, assessments and transitions.
  • Support hospital discharge and admission avoidance.
  • Develop sustainable, community-based support packages.
  • Lead multidisciplinary assessments and specialist input.
  • Present recommendations to funding panels and senior stakeholders.
  • Support service mobilisation, pathway development and occupancy growth.
  • Produce high-quality assessments, reports and recommendations.
  • Promote person-centred, least restrictive and outcome-focused practice.

Elevation Recruitment Group are recruiting on behalf of a leading specialist care organisation for a newly created Complex Assessments Manager position.

This is a fantastic opportunity for an experienced Senior Practitioner, Assessment/Referrals Manager, Nurse, Social Worker, PBS Practitioner, Clinical Professional, Operational Manager or Commissioning Professional to take on a strategic role with genuine influence over service development and patient outcomes.

You'll lead the assessment, progression and mobilisation of complex referrals across Learning Disabilities, Autism, Mental Health and Complex Care, working closely with NHS partners, commissioners, Local Authorities and multidisciplinary teams.

The Opportunity

You'll play a key role in:

  • Managing complex referrals, assessments, admissions and transitions
  • Supporting hospital discharge and admission avoidance
  • Developing sustainable, community-based support packages
  • Leading multidisciplinary assessments and specialist input
  • Managing risk, safeguarding and placement stability
  • Building strong relationships with commissioners, NHS Trusts, ICBs and Local Authorities
  • Presenting recommendations to funding panels and senior stakeholders
  • Supporting service mobilisation, pathway development and occupancy growth
  • Producing high-quality assessments, reports and recommendations
  • Promoting person-centred, least restrictive and outcome-focused practice
About You

We're looking for an experienced Health & Social Care professional with:

  • 3+ years' experience in a senior practitioner, assessment, referrals, admissions, commissioning, operational or clinical role
  • Strong experience working with Learning Disabilities, Autism, Mental Health, Complex Care and behaviours that challenge
  • Proven experience managing complex referrals, assessments and placement planning
  • Strong assessment, analytical, risk management and report-writing skills
  • Experience with safeguarding, restrictive practices and placement breakdowns
  • Experience working with commissioners, Local Authorities, NHS Trusts, ICBs and MDTs
  • Knowledge of PBS, Mental Capacity Act, DoLS/LPS, Care Act and hospital discharge/community pathways
  • A full UK driving licence and access to a vehicle

RNLD, RMN, Social Work, OT, Psychology or PBS qualifications, alongside experience of Transforming Care, CTR/CETR, DSR or specialist services, would be highly advantageous.

Enhanced DBS and regional travel will be required.
