Job summary

Northern Healthcare is seeking an experienced Assessment Practitioner to work from the Eccles office with regular travel across Greater Manchester, Yorkshire, Hull, Preston, Cumbria and the Midlands. You will conduct biopsychosocial assessments and provide clinical recommendations to support safe, person-centred admissions.

The role emphasises independent clinical judgement, collaborative working with partners, and ensuring high standards of safeguarding and legislative compliance.

Qualifications

• RMN, Occupational Therapist or Social Worker with current professional registration.
• At least 2 years' post-registration experience within mental health, learning disability or autism services.
• Strong clinical assessment, formulation and decision‑making skills.
• Experience completing complex risk assessments.
• Knowledge of the Mental Health Act, Mental Capacity Act and safeguarding legislation.
• Experience working collaboratively within multidisciplinary teams.
• Excellent written communication and report‑writing skills.
• A full UK driving licence and willingness to travel regularly across our services.

We're now looking for an experienced Assessment Practitioner to join our growing clinical team. About the role

As an Assessment Practitioner, you’ll work autonomously across our services, completing comprehensive assessments and providing clinical recommendations to support safe, effective and person-centred admissions.

You’ll work alongside commissioners, care coordinators, families, hospitals, community teams and our operational colleagues, bringing together different perspectives to make informed decisions.

No two assessments will be the same. You’ll work with people from a wide range of backgrounds, with different experiences, strengths and aspirations.

You’ll need to be confident making professional judgements, managing complexity and balancing risk with the importance of choice, independence and positive risk‑taking.

Your responsibilities will include:
