Responsibilities

• Assess, plan, deliver and evaluate personalised care plans.
• Lead, supervise and support a team of care staff.
• Administer and manage medicines safely.
• Provide nursing support in line with NMC guidelines and best practice.
• Monitor health and wellbeing and respond to changes.
• Support daily living tasks and promote independence, dignity, and choice.
• Liaise with families, health professionals and other agencies.
• Maintain accurate records and comply with regulatory requirements.
