Job summary

City Health Care Partnership CIC is seeking a care support worker to help deliver person-centred rehabilitation care, supporting patients to maintain wellbeing, independence and quality of life.

Working with a caring team you’ll carry out care activities, monitor observations, and assist with equipment and stock, while recording information and supporting basic admin tasks to ensure safe, dignified care for every patient.

Qualifications

• Level 2 qualification in Adult Health and Social Care or equivalent.
• Experience in healthcare/homecare/care home settings.
• Willingness to complete the Care Certificate during induction.
• Strong communication and interpersonal skills.

Responsibilities

• Support patients with care activities in line with individual care plans.
• Carry out observations, blood pressure monitoring and specimen collection as trained.
• Communicate effectively with patients, carers and healthcare professionals.
• Record patient information on electronic systems and assist with basic admin tasks.
• Help maintain a safe, organized environment and manage equipment/stock.
