Job summary

Sarah Bush Lincoln seeks a Hospice Aide/CNA for the Hospice House, PRN shifts from 6am to 6pm. The role provides hands-on personal care, adherence to the patient Plan of Care, and collaboration with the RN and interdisciplinary team to support hospice patients.

Responsibilities include assisting daily living activities, documenting observations, attending trainings, and contributing to a safe environment. A valid Illinois driver’s license and CNA certification are required; RN program first

Qualifications

• CNA certification or RN program first semester.
• Valid driver’s license (Illinois).

Responsibilities

• Assists in the implementation of the patient Plan of Care.
• Assists patients with activities of daily living.
• Attends in-services, staff meetings, and continuing education.
• Follows verbal and written directions provided by the RN, specific to the patient assigned.
• Maintains a clean and safe environment and performs light housekeeping assignments as assigned.
• Maintains and submits documentation as required. Records observations, completed, and refused tasks.
• Observes, records, and reports significant changes in the status of a patient and/or family to supervisor.
• Participates in interdisciplinary team meetings.
• Prepares light meals if assigned in patient plan of care.
• Provides hands-on personal care to patients.
• Provides personal care and supportive services to hospice patients.
