Job summary

You will undertake delegated clinical tasks, record observations, support care plans, promote dignity and independence, and contribute to timely, person-centred care within the Gateshead area.

Qualifications

• GCSE English or equivalent required.
• Care certificate or level 2/3 diploma in health and social care required.

Responsibilities

• Assist with delegated clinical care duties in hospital or community.
• Record patient observations and report changes.
• Support individuals with activities of daily living and personal hygiene.
• Maintain therapeutic relationships and contribute to care plans.
• Follow trust policies and document information in electronic records.
