Destinydot.com is seeking a Health Care Assistant/Support Worker in Oakham to provide compassionate in-home or facility-based care. You will follow care plans, support daily tasks, document progress in IT systems, and coordinate with healthcare teams.

The role requires strong English communication, caring nature, and ability to manage multiple tasks. Visa sponsorship is available for the UK.

Formación

• Experience in home care or care facilities.
• Ability to follow and implement individual care plans.
• Strong English communication with clients and colleagues.
• Excellent interpersonal and caring disposition.
• Capacity to manage multiple tasks and stay organized.
• Familiarity with healthcare IT systems for recording progress.

Responsabilidades

• Assist clients with daily activities such as eating, dressing, bathing, and mobility.
• Contribute to developing and executing individualised care plans.
• Maintain clear and timely communication with patients, families and medical staff.
• Utilise IT to update care records and document observations.
• Safely transport clients to appointments when needed and as per guidelines.
• Observe health status changes and report to healthcare providers.
• Follow medicines administration guidance and maintain hygiene standards.
