Key duties include coordinating care plans, acting as primary contact, scheduling appointments, maintaining records, and collaborating with multidisciplinary teams to adapt services and ensure high-quality, compliant care.

Qualifications

• Proven office experience is desirable.
• Excellent organisational and communication skills.
• Ability to work independently and as part of a team in a fast-paced setting.
• Proficiency in MS Office applications (Word, Excel, Outlook).
• Interest in healthcare or social services is advantageous.

Responsibilities

• Coordinate and oversee individual care plans, ensuring all aspects of client needs are met.
• Act as a primary contact for clients, families, and healthcare providers.
• Schedule appointments, follow-ups, and service deliveries.
• Maintain accurate records of client information and progress notes using organisational software.
• Assist with administrative tasks such as filing, data entry, and managing correspondence.
• Collaborate with multidisciplinary teams to adapt care plans as needed and ensure continuity of care.
• Monitor service delivery to uphold high standards of quality and compliance with relevant policies.
• Conduct personalised conversations to understand individual needs, preferences, and goals.
