A leading mental health services provider in England is seeking a candidate to lead the community transformation program. The role involves managing a multi-disciplinary team and engaging with stakeholders to enhance mental health services within the community. Ideal candidates will have experience in program management and a commitment to inclusive practices.
Aligned to the NHS Ten year health plan, the opportunity to deliver a comprehensive change to the way the Trust works with its partners to deliver care, providing integrated mental health services that are inclusive and accessible, supporting people's mental health needs and enabling them to live well in their community.
The postholder will lead a large, multi-disciplinary team, to implement the agreed transformed community care model, whilst needing to flex the approach to account for locality differences in demand and staffing. This will involve working with operational and clinical leads from across the Trust, and working closely with people with lived experience, carers, staff representatives and commissioners.
