Job summary

County Durham and Darlington NHS Foundation Trust seeks staff to provide support to Therapists across community settings, enabling high quality interventions for patients requiring physiotherapy, speech and language therapy or occupational therapy in the community.

The role involves holistic care, working within a multi-disciplinary team and contributing to service improvement, with emphasis on completing pre-employment checks and presenting essential certificates.

Qualifications

• Must provide all certificates stated essential in the person specification.
• Must be able to complete pre-employment checks.

Responsibilities

• Provide support to Therapists in the community to deliver high quality interventions.
• Assist in the delivery of core programmes of clinical care and participate in service improvement initiatives.
• Work within a multi-agency, multi-disciplinary framework with nursing, social work, therapy, residential, administrative and support staff.
• Care for stroke survivors in their homes or residential homes requiring care and support in the community 24 hours a day, 365 days a year.
