Are you a passionate, and caring nurse, with significant management experience, who can oversee the provision of high-quality holistic care for people in their own home with acute, complex, and long-term conditions? Experience of working in a community setting is an advantage.

Main duties of the job

A vacancy has arisen within the Unplanned Care Team, which has an innovative approach to nursing care. It involves quick responses to patients in their home to assess, plan, manage and evaluate the presenting issue, prevent unnecessary hospital admissions, and help to facilitate early hospital discharges.

You will be managing a team of qualified community nurses, nursing associates and health care support workers, and Advanced Practitioners, to take an innovative solution-focused approach to challenging situations. You will be supported by your peers, and a robust management structure.

We are looking for a highly motivated individual who is interested in both meeting the challenge of operational management and providing clinical care for patients in their home. As part of the Unplanned care team, you will be working with a variety of disciplines within the multidisciplinary team to achieve the best outcomes for patients. You'll be working on a rotational basis as part of a seven-day service covering 24-hours-a-day with the support of agile technology to help you deliver timely, high-quality care.
