Community Support Worker - Young People with Disabilities Service

Permanent, Full Time

£24,395 to £31,131 Per Annum

Location: Chelmsford

An opportunity has arisen to join the Young People with Disabilities Team based in County Hall, Chelmsford.

Working days and hours are Monday - Thursday 9am to 5:30pm and Friday 9am - 4:30pm.

Children and Young People with Disabilities - YPWD Team (16 - 25 years old)

The Young People with Disabilities Team (YPWD) works with young people and their families from the age of 16 up to 25. The team is part of the Children with Disabilities Service, which supports children from 0-25 years old with disabilities. This service collaborates with the Children with Disabilities (0-16), Preparation for Adulthood Advisors, and Occupational Therapy Teams. It is a vibrant service undergoing continuous improvement to enhance outcomes for children and young people with disabilities and their families, embracing SEND reforms.

The Role

We are seeking a Community Support Worker for the Young People with Disabilities - Mid team, based at County Hall, working in Chelmsford and surrounding Mid Essex areas. The role is for working with individuals aged 18 and above. The worker will collaborate with partners to deliver early help, family support, and social work interventions that build resilience, remove barriers, and foster a positive future.

Support Workers support frontline teams by providing direct activity, one-to-one or group work, offering practical, personal, and emotional support aligned with assessed needs to promote health, development, and well-being.

Legislation such as the Care Act 2014, Mental Capacity Act 2005, and other relevant laws support this role within the social care framework.

Accountabilities

  • Assist social workers and occupational therapists with activities and interventions supporting the plans for children, young people, and families.
  • Develop strong, effective relationships with children, young people, and families, promoting strengths-based social work practices and maintaining professionalism to enhance confidence in Children's Social Care.
  • Highlight safeguarding issues and support the implementation of safeguarding plans.
  • Collaborate with internal and external agencies (e.g., Occupational Therapy, Schools, health services, mental health providers) to gather information for assessments and decision-making.
  • Provide information and advocacy regarding third-party agencies (benefits, immigration, housing) as needed.
  • Participate in meetings, reviews, case conferences, and court hearings to inform understanding of needs.
  • Record work activities accurately in accordance with policies, including case records on Mosaic.
  • Work collaboratively across teams and agencies to achieve positive outcomes for children and families.
  • Deliver support as per the plan, with measurable impacts, typically within time-limited periods.
  • Make analyses and recommendations regarding care and support packages based on assessments under the Care Act 2014.

The Experience You Will Bring

Essential:

  • Educated to RQF Level 3 (A Level/NVQ/BTEC) or equivalent experience. Ability to undertake Care Act assessments and Support Plans is desirable but not mandatory.
  • Ability to write and review care plans.
  • Evidence of ongoing professional development and deep knowledge in relevant areas.
  • Experience in building strong relationships with children/young people with disabilities and their families, including during challenging times, and experience in direct and group work.

Desirable:

  • Knowledge of issues affecting children, young people, and families requiring support.
  • Understanding of the Care Act 2014, Mental Capacity Act 2005, and other relevant legislation and policies.
  • Proven experience working with partner agencies and external organizations.
  • Experience in undertaking adult assessments (desirable).
