Are you looking for an exciting opportunity to work within a well-established specialist community learning disability and autism team?

Greater Manchester Specialist Support Team is seeking a support worker to join our team across Greater Manchester. We welcome applications from enthusiastic and confident individuals committed to the wellbeing of service users and delivering high-quality services. Our team focuses on enhancing safety, experience, and wellbeing in the community.

The team is multidisciplinary, including Nursing, Speech and Language Therapists, Occupational Therapists, Social Workers, Psychologists, and Psychiatrists. You will work across Greater Manchester, liaising with partner agencies such as Health, Social Care, and Criminal Justice to develop proactive risk mitigation and support strategies.

The primary role involves carrying out duties related to direct care and observation of service users under supervision. You will report observations on mental state and risk factors to qualified staff and work with service users individually and in groups as per care plans and directives from lead professionals.

Key Responsibilities:
  1. Coordinate assessment and planned interventions with guidance from qualified practitioners.
  2. Implement treatment and care plans, engaging service users to develop social, self-help, occupational, and personal skills.
  3. Assist with daily living activities, social, recreational, and occupational activities.
  4. Function as a key worker under the direction of a lead clinician.
  5. Report observations and concerns regarding physical or mental health to qualified practitioners.
  6. Communicate effectively across departments and agencies.
  7. Use verbal and non-verbal skills to overcome communication barriers.
  8. Maintain a safe environment for service users in the community.
  9. Assist in assessments as requested by qualified staff.
  10. Develop links with statutory and voluntary agencies.
  11. Support social care staff in delivering care according to plans, acting as a role model.
  12. Work hours are 9-5, Monday to Friday, with flexible options.
  13. Use de-escalation techniques to manage aggression.
  14. Work directly with service users in their homes, including assessments, interventions, and crisis support.
  15. Facilitate group interventions and support staff training development.
  16. Maintain accurate documentation and contribute to risk management planning.
