NHS is seeking a dedicated Community Support Worker for its Integrated Referral Hub. In this role, you will respond to non-injured patients and assist them in regaining independence at home. Join a progressive team that offers opportunities for growth and a supportive work environment.
We have an exciting opportunity to come and join our expanding Community Integrated referral hub team.
The role of Community Support worker is an integral part of the CIRH team, as a Community Support Worker, you will be the first responder to the patient and will provide care and support to non injured patients who have fallen in their own homes. By first response, we simply mean that you are the first person there to support.
As this is a new and exciting role, there will be plenty of opportunity to grow and develop yourself and your skill set.
As we are a 7 day service, the hours will be worked across those 7 days on a rota basis. You will be integrated within the team to meet the demands of the business.
Hours and days can be flexible to support the candidate, this can be discussed at interview.
Once a call has been made to our Community referral hub (CRH) you will be required to attend the patients home within a 45 minute target time.
