Qualifications

• Previous experience as a Support Worker, Healthcare Assistant or equivalent care role is preferred.
• Caring, patient and person-centred approach.
• Good communication and interpersonal skills are essential.
• Ability to work independently as well as in a team.
• Strong safeguarding knowledge and professional boundaries.
• Flexibility and reliability when accepting shifts.

Responsibilities

• Provide person-centred care and support in community settings.
• Assist service users with daily living activities to maintain independence.
• Follow individual care and support plans and monitor progress.
• Support appointments, activities and community engagement for service users.
• Maintain accurate records and complete relevant documentation.
• Collaborate with colleagues and other professionals to ensure quality care.
