A local council in England is seeking a Community Support Worker to support individuals in accessing resources and conducting assessments under the Care Act 2014. This role involves visiting clients in various settings, developing support plans, and ensuring the safety of individuals. The position offers a flexible hybrid working model and requires a Level 2 in Maths and English, along with relevant social care experience.
As a Community Support Worker, you will meet with individuals, families and carers to connect, listen and, where necessary, undertake an assessment under the Care Act 2014. Where there is an eligible need, you will devise a plan for how the person can access the support they require. This may include connecting with their local community, referral to other agencies, or providing services to support unmet needs. You will identify and report safeguarding concerns, consider mental capacity, and other high-risk situations where a person’s safety or liberty may be at risk. Your team will have an office base and can work in a hybrid way. You will undertake visits to people in their own homes, in a hospital setting or wherever the person feels most comfortable to meet you.
The Accommodation Review team completes annual reviews for people living in residential care, supported living and shared lives services. They complete Care Act assessments, Mental Capacity assessments and assess eligibility, and work closely with the fee review team to complete care cubed assessments and annual care uplifts.
As a Community Support Worker in the Domiciliary Review Service, you will work directly with individuals, families, and carers to build connections and actively listen to their experiences. Your primary role will be to carry out reviews of the support currently in place to ensure it continues to meet the person’s eligible needs. This involves collaboratively reviewing existing Support Plans to confirm they are delivering the intended outcomes. Where ongoing eligibility is confirmed, you will update the Support Plan to make the best use of available community resources and broaden opportunities for the individual. This may include making referrals to other services and working closely with a range of professionals and organisations. You will also be responsible for identifying and reporting any safeguarding concerns, assessing mental capacity where appropriate, and responding to high-risk situations that may impact a person’s safety or liberty. The team operates from an office base with the flexibility of hybrid working. You will carry out visits in people’s homes or other locations as agreed with the individual. Additionally, you will work proactively with carers to ensure their needs and any impacts are recognised and addressed during the review process. Both the Accommodation and Domiciliary teams, although based in Glenfield, cover the whole of Leicestershire as the positions operate county wide including with out of county funded placements.

As a Grade 8 Trainee Community Support Worker you will be in your first CSW role and will be expected to develop experience, knowledge and skills, complete the Learning and Induction Programme and undertake your duties with support and supervision. As a Grade 9 Community Support Worker you will be expected to have sufficient experience, knowledge and skills to manage your workload under supervision, to deliver a high quality, person-centred, accountable statutory social care. This includes demonstrating competency undertaking Care and Support Assessments, Mental Capacity Assessments and Best Interest decisions.
