Job summary

Herefordshire and Worcestershire Health and Care NHS Trust is seeking a Community Support Worker in Leominster. The role is NHS AfC Band 3, 30 hours per week, pro-rated for part-time, with a Monday-Friday 09:00–17:00 pattern and no weekends.

You will provide recovery-focused mental health support within a neighbourhood team and contribute to high-quality community care. You'll work with MDTs, support service users and carers, and have opportunities for professional development within a

Job overview

Come and join ever-expanding, friendly, and supportive Neighbourhood Mental Health Team, who pride ourselves in providing outstanding recovery-focused mental health care to patients across the North and West Locality of Herefordshire. 30 hours over 4 days a week Monday-Friday 09:00hrs to 17:00hrs. No weekends or Bank holidays

Main duties of the job

Work as a key member and resource of the multidisciplinary (MDT) core team.

Aspects of the role would also include close working within the MDT, undertaking and supporting service users accessing the care of the team for recovery-focused interventions.

There is scope for professional development as the Trust has a repertoire of training available to those wishing to extend their skill set. Developing our workforce is important to us, and we are pioneering champion roles across the organisation.

Support registered professionals to deliver evidenced based interventions, managing a caseload of patients.

Supporting and signposting to community-based services to improve social isolation and mental wellbeing.

You will have experience of working with individuals who have mental health difficulties.

The ability to work flexibly as the need of the service demands is essential.

Attend weekly MDT meetings, participate in professional’s meetings, refer and signpost to appropriate services dependent on patient need to improve overall recovery journey.

Provide support and guidance to the carers of patients accessing the services.
