The post is 37.5 hours per week, Mondayto Friday and initially is a fixed term contract for 18 months.
Following the induction period, you will be working within thecommunity and travelling across the catchment area, which is quite extensive.
The Droitwich, Ombersley, and the RuralsNeighbourhood Mental Health Team has grown significantly since the NHS EnglandMental Health Transformation model was implemented in 2021, where mental healthservices have been situated in the heart of the community that they work with.

Work as a keymember and resource of the multidisciplinary (MDT) core team.
Aspects of therole would also include close working within the MDT, undertaking andsupporting service users accessing the care of the team for recovery-focusedinterventions.

Supportregistered professionals to deliver evidenced based interventions, managing acaseload of patients.
Supporting andsignposting to community-based services to improve social isolation and mentalwellbeing.
You will haveexperience of working with individuals who have mental health difficulties.
The ability towork flexibly as the needs of the service demands is essential.
Attend weekly MDT meetings, participatein professionals meetings, refer and signpost to appropriate services, dependenton patient need to improve overall recovery journey.
Provide support and guidance to thecarers of patients accessing the services.
