Community Support Worker - Reablement
The community support service supports people to regain their independence through reablement. We are a CQC registered service that is rated overall 'Outstanding'.
Our reablement team works with individuals to increase their ability to look after themselves following illness or injury and to rebuild the skills and confidence to help them live as independently as possible. We work closely with our occupational therapy and social work colleagues to support people coming home from the hospital and assist those in the community to maintain their independence.
You will play a critical role in identifying meaningful and achievable goals that matter to the person. This, in turn, will promote good health, wellbeing, a sense of purpose, and self-worth. You will become a valued member of the team that helps people to direct their own recovery and reduce the risk of them returning to the hospital.
The Community Support Service is seeking to recruit flexible, motivated, and enthusiastic individuals who feel they can make a difference to vulnerable people living in their own homes.
You will need to have a full driving license and access to a car when at work.
We can offer a contract of guaranteed hours.
