Max Emerald Ltd in Colchester is seeking a Community Support Worker for an Admission Avoidance project. This temporary 6-month role supports adults with complex social needs to live more independently.

You will assess needs, work with local teams, and help develop care plans while safeguarding vulnerable clients and promoting prevention.

The role requires strong relationship-building, willingness to learn, and collaboration with carers, housing officers, and therapists to keep people safe.

Qualifications

• Strong ability to build effective relationships with adults and their families in times of challenge and crisis.
• Drive and passion to make a difference, and a willingness to learn.
• Experience of activity-based work with adults in a social care setting.
• Knowledge of local communities.

Responsabilités

• Work with neighbourhood teams to support admission avoidance.
• Undertake Care Act Assessments and commission services to meet long-term needs.
• Collaborate with the Occupational Therapy hub and pursue approved assessor training or development.
• Coordinate with Discharge to Assess and Transfer of Care Hub at Colchester Hospital.
• Foster collaboration with community services including professionals from non-social care services and carers such as community pharmacists, housing officers and dementia support teams.
• Support individuals not eligible for ASC commissioned services to manage their own social care needs and emphasise prevention.
