A healthcare provider in Coventry is seeking a Community Stroke Rehabilitation Assistant to develop and deliver therapy treatment programmes for patients. The role involves effective communication, patient education, and evaluating patient progress while working collaboratively with a multidisciplinary team. Experience with diverse patient needs and competency in clinical practice are essential. This position provides an opportunity to improve patient care and engage with various therapy assistants.
Clinical: To gain informed consent for treatment in accordance with Trust policy. This will frequently be to patients/carers and relatives who may have diverse or complex emotional, physical, and psychological conditions, e.g., deafness, dysphasia, depression, chronic pain, etc., requiring the use of verbal and non-verbal communication tools. To develop and deliver therapy treatment programmes using guidelines set by qualified therapists, such as patient education, exercise programmes, or classes. Evaluate patient progress, reassess, and modify treatment programmes as necessary after discussion with qualified therapists.
To ensure own competency prior to using any medical device (e.g., gym equipment, hoists, mobility aids) and when issuing equipment to patients (e.g., appliances, splints, aids). Maintain accurate documentation both written and electronic in accordance with professional, legal, and Trust guidelines. Responsible for managing own caseload within a defined area of the service and working without direct supervision.
Maintain a clean, tidy, and safe environment and equipment in Trust premises or clients' homes, respecting Health and Safety guidelines. Apply clinical knowledge and experience to assess and manage clinical risk within the scope of practice. Work within infection control guidelines to prevent infection transmission. Support other therapy assistants as required. Collect relevant clinical data as needed.
Communication: Effectively communicate with patients, carers, and relatives, using verbal and non-verbal methods to maximize rehabilitation potential. This may involve explanation, instruction, persuasion, motivation, negotiation, and reassurance. Commit to a team approach to ensure continuity and efficiency. Liaise with multidisciplinary team members, colleagues, and medical/nursing staff as required. Carry out therapy work within patients' home environments.
Other: Order general store articles and equipment needed by the department, maintaining adequate stock levels in coordination with the manager. Perform administrative duties such as data input, typing, filing, and reception cover, working collaboratively with clerical staff. Attend meetings as designated by senior therapists. Identify opportunities to improve the service for better patient care within available resources.
