The post will involve working with healthcare assistants and clinicians to provide nursing interventions to patients in their home environment, maximising their independence and providing care to avoid hospital admission as well as facilitating discharges from the acute.
You must be innovative in your approach and willing to work with other members of the integrated team and ward staff to ensure a high-quality service delivery. You must be willing to develop your own knowledge and skills; you will be supported in your personal development with training to extend your community nursing skills.
Experienced and newly qualified staff nurses will be considered for the post. 7 days a week shift pattern.
