Overview

An exciting opportunity has arisen for a registered nurse to join our forward thinking and award winning Care Community Team in Winsford, to cover a 7 day service. There are full (37.5 hours per week) and part time (22.5/30 hours per week) posts available.

Responsibilities
  • Assess nursing care needs, implement and evaluate care programmes and deliver high quality holistic care within the community setting and in wound care clinics.
  • Support peers, the team leader and the organisation in promoting the Trust’s values and behaviours.
  • Work independently with good time management and organisational skills; be adaptable, reliable and approachable; and be willing to work in other areas to meet service needs.
  • Travel is an essential part of the role; demonstrate on the application how you would meet this requirement.
  • Support patients within the Winsford Care Community Team, which includes Complex Case Practitioners, District Nurse/Case Managers, Macmillan and Therapy teams.
  • Holistically assess patients’ needs, ensuring health promotion and quality improvement to meet patient needs; utilise modern IT software applications.
  • Maintain a structured induction, with a mentor during induction and probation, and develop competencies within a supportive team environment.
  • Assess and manage health needs of identified populations; develop, implement and evaluate care programmes including end of life care and patients with long-term conditions and complex nursing needs.
  • Promote patients’ wishes to remain in their preferred place of care, maximise independence and quality of life; liaise with healthcare professionals and other agencies to ensure effective service delivery.
  • Undertake initial assessments on receipt of referrals; plan, implement and evaluate care plans addressing physical, social, emotional and environmental needs (rehabilitative/self-care where appropriate).
  • Provide skilled nursing care and clinical tasks including wound care, leg ulcer assessment, venepuncture, catheter care, medication administration (where applicable) and management of intravenous lines.
  • Recognise, interpret and analyse clinical data; teach and supervise nursing care that can be safely delegated to patients/carers.
  • Promote clinical excellence with evidence-based care and maintain competencies in line with the competency framework; work within Trust policies and procedures.
  • Contribute to End of Life (EOL) care, and participate in health education for individuals and the community to promote self-care.
  • Accurately record all patient contacts on EMIS Web and Trust documentation, ensuring confidentiality in line with NMC guidelines.
  • Manage discharge potential and needs from the caseload; ensure equipment and aids meet patient needs and are maintained; demonstrate safe use of equipment as required.
  • Participate in care delivery for individuals with complex needs and support carers and relatives; refer to other practitioners when needs exceed scope of practice.
  • Prioritise and manage own caseload; assume responsibility for the clinical caseload in the absence of the Team Leader/Senior Lead and delegate work within the competency framework.
  • Participate in quality monitoring and, if a qualified nurse prescriber, participate in Non-medical Prescribing following evidence-based assessment and the wound care prescribing formulary.
  • Maintain accurate personal and professional records, ensuring efficient and cost-effective use of resources; report accidents/incident events via the incident reporting system and participate in investigations as necessary.
  • Co-operate in coordinating off-duty and holiday leave to maintain service cover; support other team members in all aspects of the service.
  • Engage with the Integrated Governance Strategy and participate in appraisal and clinical supervision as per Trust policy. Maintain privacy, dignity and confidentiality in all interactions.
