An exciting opportunity has arisen for an aspiring individual to develop their career in the specialised area of District nursing. This opportunity is now available in the Chalfont's and Gerrards Cross area to become a Community Nurse within a newly formed team. We are welcoming highly motivated and effective team players, committed to providing a high-quality nursing service within the District Nursing field. As a community nurse, you would be supported to develop into an autonomous practitioner; nursing patients with multiple complex health care needs in their own home.

Main duties of the job

Community nursing is a specialised area of practice. As a Community Staff Nurse, you will work within a locality team of District Nurse Team Leaders, Community Staff Nurses, Therapists, Assistant Practitioners and Healthcare Assistants, to meet the needs of the complex patient in the community and avoid admissions to acute settings. This includes medication administration, end of life care, managing the deteriorating patient, complex and simple wound care including negative pressure wound therapy and compression therapy, venepuncture, utilising advanced assessment skills and clinical nursing skills. We nurse people with a variety of conditions in a holistic manner in their own home and in residential care settings. Community nurses work in close partnership with social care and other primary health care professionals such as GPs, Community Therapies and specialised services such as Tissue Viability, Diabetes and Heart Failure.

Person Specification

EDUCATION, QUALIFICATIONS & TRAINING

  • Registered Nurse or student nurse awaiting confirmation of NMC registration.
  • Committed to own professional development to enhance practice.
  • Post-registration education and extended skills e.g., Intravenous drug administration, compression dressings.
  • Hold / be working towards an NMC approved mentoring qualification.

SKILLS, ABILITIES & KNOWLEDGE

  • To be able to communicate effectively with patients, relatives, carers, and other professionals.
  • To be able to care for patients without prejudice.
  • Able to work under pressure, self-aware, self-motivated, enthusiastic.
  • Ability to manage own caseload.
  • Knowledge of the NMC Code of Professional Conduct.
  • Knowledge of the Data Protection Act.
  • Ability to maintain a safe clinical environment conducive to patient care. Adaptable to change.
  • Ability to work rotating shifts.
  • Diplomatic approach with professionalism.
  • Awareness of NHSE guidance on clinical governance to continuously improve quality of care (e.g., quality, safety, and ward audit standards).
  • Knowledge of care planning, preceptorship, reflection, and clinical supervision.

EXPERIENCE

  • Demonstrate experience post or pre-registration placements in relevant clinical areas: care of elderly, community, orthopaedics, neurology, respiratory, palliative care, long-term conditions.
  • Desirable but not essential knowledge of working with a single assessment, planning, and delivery of care.
  • Experience of electronic clinical systems and technology e.g., RIO.

SPECIAL CIRCUMSTANCES

  • Car driver/owner able to travel to other locations within the trust.

  • Must have rights to work in the UK.

£29,970 to £36,483 a year, per annum, pro-rata.
