Job summary

Buckinghamshire Healthcare NHS Trust in High Wycombe is seeking two Band 5 Community Staff Nurses to join our new community service, delivering planned care in patients’ homes across seven locality teams.

This predominantly proactive, community-based role focuses on supporting patients to manage their health and wellbeing at home, with opportunities to provide long-term condition reviews, continence assessments and phlebotomy, and to supervise Healthcare Assistants for safe, effective care.

Qualifications

• Excellent verbal and written communication.
• Ability to work in a team and on own initiative.
• Post-registration nursing experience and ongoing professional development.

Responsibilities

• Deliver planned patient care in the community setting.
• Manage a caseload with supervision and evaluation of service delivery.
• Support long-term condition reviews, continence management and phlebotomy.
