South Tees Hospitals NHS Foundation Trust is seeking a Community Staff Nurse attached to the East Cleveland PCN. You will provide coordinated care through integrated community teams and work closely with GP practices and partners to support patients with complex needs, aiming to prevent hospital admissions.
Successful applicants will lead and manage the community nursing workforce within the PCN, embracing NHS Long Term Plan aims and new care models.
Formación
- Registered with the NMC to practice in the UK.
- Experience in acute or community settings is desirable.
