Community Staff Nurse in School Nurse Team- Elmbridge

The Community Staff Nurse in the School Nurse Team is a registered general nurse working as part of the public health nursing team to provide preventative and supportive health care to children, young people, and their families.

The post holder works with a public health focus under the supervision of a Specialist Community Public Health Nurse (SCPHN) with a defined population.

As part of the public health nursing team, the post holder collaborates with a wider multi-disciplinary and inter-agency team to offer public health services to the community.

The work takes place in a variety of settings, including home, clinic, and community venues.

Base

Our Healthy Family Teams support our Surrey-wide service. You will be based at one of our Office Hubs across Surrey with the opportunity for hybrid working.

Main duties of the job
  1. Deliver the healthy child programme at four levels as delegated by the Specialist Community Public Health School Nurse.
  2. Recognise health needs for individual clients and situations concerning the health of children, young people, individuals, and families, sometimes in complex situations, and identify when referral for support from Public Health Specialist is necessary.
  3. Undertake health assessments in schools.
  4. Prepare for, attend, and participate in relevant case conferences and inter-agency meetings as delegated by the SCPHN. Undertake health needs assessments and care planning as appropriate.
  5. Support the SCPHN in providing specialist targeted support to children identified as at risk or with complex needs, ensuring Partnership Plus plans are in place.
  6. Assist the SCPHN in identifying, prioritising, and implementing programmes of support responding to public health needs of the school-age population.
  7. Conduct comprehensive assessments of individual public health needs and provide support or onward referral as appropriate, focusing on sexual health, smoking, and weight management.
  8. Offer advice and support to children, young people, and families based on evidence-based practice.

Essential:

  • Registered Nurse qualification
  • Child Protection awareness
  • Knowledge of national and local policies in Children’s Services and Public Health
