Job description

This role is not eligible for visa sponsorship. Applications requiring sponsorship may be withdrawn. An exciting opportunity has arisen for a NMC Registered Band 5 Nurse to join the Home First Team (Discharge to Assess/D2A Service) in North Staffordshire.

The team supports patients in the community to prevent hospital admissions or assist with discharge following acute hospital care. We deliver high-quality care to individuals with complex needs, promoting independence, rehabilitation, and self-care. Our ethos is to enable safe discharges and provide nursing support at home, helping patients remain in their own environment and avoid unnecessary readmissions.

As a valued team member, you'll work collaboratively within a multi-disciplinary team and with Operational Leadership to ensure excellent care delivery. You'll carry out specialist assessments and care planning with service users and carers. This role involves working shifts across a 24/7, 365-day schedule and requires independent travel across a large geographical area. You may be required to work from any of the three North Staffordshire bases.

As a Band 5 Nurse you will provide holistic assessment, planning, implementation and evaluation of care as part of the D2A process. Your responsibilities will include:

  • Acting as a role model and motivating team members in working towards innovative and high standard of practice.
  • Participating and supporting the Senior Nurse Coordinator and Clinical Lead in the development of an effective team and the development of productive working relationships throughout the Trust.
  • Supporting the Senior Nurse Coordinator and Clinical Lead in the development of a positive and 'supportive' team culture.
  • Providing public health, health promotional programs to improve health and reduce inequalities.
  • Being responsible, and accountable, for service delivery to clients/patients.
  • Demonstrating at all times the Trust Values.
  • Improving health and wellbeing, including mental health.
  • Facilitating early supported discharge.
  • Initiating referrals to other health professional specialist services and agencies, and establishing and monitoring good working relationships with other health and social care professionals, statutory and voluntary organisations.
  • Providing patients and relatives with information and education, ensuring they have meaningful choices that promote dignity, independence and quality of life.
  • Advising the Senior Nurse Coordinator and Clinical Lead of any issues that may affect the patient or the service.

In return for your hard work and dedication, we offer a range of benefits, including:

  • Supporting your career development and progression.
  • An excellent NHS Pension scheme.
  • Generous maternity, paternity and adoption leave.
  • Options for flexible working.
  • Up to 27 days annual leave (increasing with service up to 33 days) and the opportunity to purchase additional leave.
  • Extensive Health and Wellbeing support and resources.
  • Reimbursement for travel expenses between patients.
  • A lease car scheme (if you complete more than 500 business miles per annum), fully insured and maintained.
  • A salary sacrifice car scheme, fully insured and maintained.
  • A salary sacrifice bike scheme (up to £2k).
  • Free car parking at all trust sites.
  • Free flu vaccinations every year.
  • Citizens Advice support linked with a Hardship Fund for one-off additional support (up to £250, if the criteria is met).

We are proud to be a diverse and inclusive organisation, and there are a range of staff networks available to help you meet like-minded people.
