A community nursing service is seeking skilled candidates to deliver compassionate nursing care in patients' homes and clinics. We are looking for individuals who can be part of our fast-moving, innovative, and ambitious teams, dedicated to supporting our communities to manage their own health both at home and in our dedicated clinics.
You will be responsible for providing skilled nursing care to patients across the locality and work flexibly with multidisciplinary teams across our service to support our patients' journey.
You will need to demonstrate excellent communication and interpersonal skills and be adaptable, reliable, resourceful, and approachable. You’ll need to be organized and confident and able to cope with changing environments as you’ll be working independently in patients' homes, with support always available.
Please note: applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), and this will be deducted from your salary over the first 2 months of employment.
We encourage you to enroll in the DBS Update Service. An annual fee of £16 applies.
Our community teams of Nursing and Allied Health Professionals deliver care to people with significant physical health needs in various settings including clinics, community inpatient units, hospices, and patients' homes, including care homes. Our aim is to enable independence, self-care, and to help individuals live their optimum lives, reducing hospital admissions.
Services range from community nursing, including specialist practitioners, to public health and health promotion such as school nursing and health visiting. Many specialist community services support care pathways like Physiotherapy, OT, Speech and Language, Dietetics, and Podiatry.
Supporting these services are administrative and managerial roles. All roles are vital in transforming community health services and ensuring high-quality care now and in the future, centered around our patients' needs. Together, we all make a difference.
