Job summary

Northumbria Healthcare NHS Foundation Trust is seeking a Community Staff Nurse to join the District Nursing Service in North Tyneside, based at Hadrian Health Centre. You will work as part of a district nursing team delivering patient-centred care in people’s homes, residential settings and clinics.

The role involves autonomous practice under supervision, assessment, wound care, palliative care, catheterisation and diagnostic tasks such as ABPI Doppler.

Qualifications

  • Must hold a full UK driving licence and have access to a vehicle.
  • Work autonomously and as part of a community nursing team.

Responsibilities

  • Assess, plan, implement and evaluate care for diverse patients in homes and clinics.
  • Provide nursing care including wound management and palliative care.
  • Support patients with terminal illness and bereavement; provide emotional support.
  • Perform ABPI Doppler tests and interpret results to decide further action.
  • Maintain high standards of professional practice and accountability.

An exciting opportunity has arisen to join the District Nursing Service in North Tyneside for a Community Staff Nurse.

We are looking for an enthusiastic staff nurse to join our district nursing team based at Hadrian Health Centre.

This is a fantastic opportunity to become a team member in an exciting service committed to providing patient centred care to patients in their own homes, residential care settings and in clinics. We are keen to provide our staff with development opportunities to enhance their skills, knowledge and career progression within the District Nursing Service, while providing high quality and safe service in line with the Trust Values.

Community staff nurses work autonomously and as part of a team, reporting directly to their line manager, the community nursing team lead, assessing, planning, implementing and evaluating care for a diverse group of patients with a wide range of conditions and health needs.

The development and delivery of programmes of health and social care.

The delivery of care and support to patients with a terminal illness and their families including the provision of emotional and bereavement support.

The performance of skilled nursing care including aspects of assessment, wound management, palliative care and catheterisation.

The performance of diagnostic tests such as Ankle Brachial Pressure Index using Doppler and the interpretation of the results to decide further courses of action.

Performance of procedures which require fine dexterity and skill such as venepuncture, drug administration by injection and the removal of sutures.

Interpretation of results and the communication of these to patients e.g. blood glucose estimation. Explaining aspects of care to patients and carers allowing the opportunity to ask questions and raise concerns.

Maintenance and promotion of high standards of professional practice including the sharing of approaches and experience with others.

Maintain a duty of care to patients, carers and colleagues through the exercise of personal and professional accountability.
