Job summary

North Tees & Hartlepool NHS Foundation Trust is seeking Staff Nurses for the Home First Response Team. The role focuses on nursing care and social support in patients0s homes, aiming to prevent hospital admission and support early discharge.

Job summary

The Home First Response Team provides nursing & social care interventions within community settings, with the clear aim of both preventing hospital admission & expediting hospital discharge. Key interventions include delivery of medications by intravenous & other routes, clinical assessment of people in the community, provision of nursing & social care packages as crisis intervention & varied other nursing duties. The post holder will have a level of knowledge in the field of community care and will have a specific interest in areas such as palliative care, long term conditions and wound care. You will be required to work autonomously to provide a whole person approach to the care and management for patients within this setting.

Main duties of the job

Home First Response is an innovative scheme providing nursing care, treatment and social support for patients in their own home preventing unnecessary unwanted hospital admission and facilitating safe early discharge.

We are looking for Staff Nurses to provide skilled nursing care to patients who access this service. Candidates will be responsible for assessment, planning, implementing and evaluating care needs and must be committed to providing high standards of patient care.

Experience of Community nursing, working within an emergency assessment unit or rehabilitation would be an advantage.

The hours of work are currently on a shift rota between 20.00 - 08.30hrs over seven days a week, pending staff consultation.

The ability to travel in the community is essential.
