As well as caring for people in their own homes, we also operate clinic environments for enhanced wound care.
No two days are the same in the friendly ST District Nursing Teams. New team members are supported with a comprehensive induction programme to The Trust and to our locality, we have a great preceptorship programme for newly qualified nurses too!

Main duties are to provide a high-quality person-centred approach to care delivery which always considers people\'s safety, privacy, dignity whilst promoting and embodying the Trust\'s values. Undertake assessments of patients care needs and develop programmes of care as part of the wider multi-disciplinary care team. As a Community Staff Nurse, you would manage acute and chronic nursing needs, be able to develop strong, therapeutic relationships with your patients and work closely with community partners to achieve the best possible health outcomes.
You\'d be supporting people to maintain their wellbeing and independence, helping to avoid unnecessary admission to hospital as well as delivering exceptional end of life care including symptom management and help in identifying and achieving people\'s preferences.

To be responsible and accountable for the delivery of safe, effective and compassionate nursing care for patients designated to their care.

To supervise non-registered staff as stipulated by the NMC under the requirements of delegation.
Provide effective leadership and assist with the management and organisation of the ward/department in the absence of the Ward manager or Junior Sister / Charge Nurse.
