Community Staff Nurse (WEBREC1000447) Frome, England

Providing outstanding care as rated by CQC, Dorothy House provides compassionate, supportive and holistic care for people in our community with a life-limiting illness. Our focus is on quality of life, helping patients to live well and die well.

We are here for anyone in our community who is facing a life-limiting illness. Using our full breadth of service, we create individual care journeys, which start at diagnosis and continue with bereavement support for family, carers and children.

The successful candidate will work as part of ourEast SomersetTeam alongside existing members of our very supportive Community Palliative Care Team and will contribute to the delivery of palliative care by providing expertise in decision making, advance care planning & holistic symptom control.

COMMUNITY STAFF NURSE | Band 5 | £29,970 - £36,483 per annum (2025 pay award pending)

The post holder will provide excellent skills in palliative care by:

  • Working flexibly as part of a very supportive team within the Dorothy House Community Team
  • Undertake holistic assessments of patients and families that have been referred to the team, this can be in the home, clinic setting or virtually.
  • To provide nursing care that is patient and family centred, acknowledging the needs of individuals and ensuring high standards of care are provided.
  • To develop skills in other community related departments within Dorothy House Hospice Care which are relevant to community services which improve the patient and family experience of Dorothy House.
  • To assess the bereavement needs of the family and refer for follow-up work with the Bereavement Service.
  • To utilise learning opportunities to enhance the knowledge and skills of health and social care colleagues.
  • To support learners undertaking appropriate health and social care courses, offering mentorship as appropriate.

HOURS OF WORK

Working in the East Somerset area.
This is a Full Time position, working 37.5 hours over 5 days.

ABOUT YOU

Due to the community based nature of the role, the post holder will need to hold a full UK driving licence with access to a vehicle.

  • First Level Nurse with relevant post basic qualification
  • Clinical experience at Band 5 level or equivalent
  • Excellent communication/interpersonal skills related to complex patient care
  • Can demonstrate experience working alongside a multi-disciplinary team
  • Ongoing evidence of professional development
  • UK driving licence with access to a vehicle
  • Personal attributes that reflect care, compassion and willingness to support patients and their families through their end of life journey
  • Clinical experience in a relevant specialty with clinical assessment and analytical skills
  • Demonstrates evidence of palliative care knowledge
  • ENB 998 or equivalent experience in teaching
  • Experience of working within the community/relevant qualification
  • Experience working independently and manage own caseload
