A leading healthcare trust is seeking a Community Staff Nurse to provide high-quality care to patients in their own homes. The role involves working with GPs and multidisciplinary teams to prevent avoidable hospital admissions. The Adult Community Service provides care to patients in their own homes and supports the prevention agenda, including care for frail older people with long term conditions. As a Community Staff Nurse you will deliver a high quality service, working closely with GPs and multidisciplinary teams to address a range of care issues across health conditions and prevent avoidable hospital admissions. We are an innovative service covering GP practices in the Stone and Eccleshall locality and seek dedicated, compassionate and highly motivated nurses. You will work independently in a community setting, participating in the ongoing assessment, planning, implementation and evaluation of programmes of care, adhering to policies and procedures.
You must be a qualified nursing practitioner with current NMC registration. Previous experience of working in the community is not an essential requirement, however you need to demonstrate a commitment to delivering high quality targeted personalised care. Post holders will be required to work flexibly on a rota system across a 7 day working week. The Trust operates a vehicle leasing scheme.
The nature of the service will be determined by local service specifications. You will be part of the Community Nursing Team and may work in a variety of settings. This is a dynamic post requiring collaboration with multidisciplinary teams to address care/support issues and improve population health.
