Job description
Overview

The Adult Community Service provides care to patients in their own homes and supports the prevention agenda, including care for frail older people with long term conditions. As a Community Staff Nurse you will deliver a high quality service, working closely with GPs and multidisciplinary teams to address a range of care issues across health conditions and prevent avoidable hospital admissions. We are an innovative service covering GP practices in the Stone and Eccleshall locality and seek dedicated, compassionate and highly motivated nurses. You will work independently in a community setting, participating in the ongoing assessment, planning, implementation and evaluation of programmes of care, adhering to policies and procedures.

You must be a qualified nursing practitioner with current NMC registration. Previous experience of working in the community is not an essential requirement, however you need to demonstrate a commitment to delivering high quality targeted personalised care. Post holders will be required to work flexibly on a rota system across a 7 day working week. The Trust operates a vehicle leasing scheme.

Responsibilities
  • Work within a Community Nursing team, contributing to the delivery of high quality evidence based treatments for patients.
  • Be competent as appropriate to the requirement of the role.
  • Demonstrate at all times the MPFT Values.
  • Have professional and clinical knowledge of community nursing.
  • Deliver care to patients in their own homes or residential homes, and/or in a clinic setting where available and appropriate.
  • Be responsible for a defined population of service users in a variety of settings across the Trust including the community and hospitals.
  • Be skilled in the assessment of care/support needs and in the development, implementation and evaluation of care/support packages.
  • Have supervisory responsibilities as required (e.g. work allocation) and may have some responsibility for service developments.
  • Be part of the Community Nursing Team providing healthcare and work in a variety of settings, addressing a range of care/support issues across the spectrum of conditions to prevent unnecessary hospital admissions and improve health and wellbeing.

  • Qualified nursing practitioner with current NMC registration.
  • Ability to work flexibly on a rota across seven days; driving and vehicle leasing arrangements may be relevant depending on locality.
  • Commitment to delivering high quality personalised care and working with uni/multidisciplinary teams.
Work Environment and Service Details

The nature of the service will be determined by local service specifications. You will be part of the Community Nursing Team and may work in a variety of settings. This is a dynamic post requiring collaboration with multidisciplinary teams to address care/support issues and improve population health.
