Are you a compassionate Registered Nurse (RN) seeking a new opportunity? We have Community Staff Nurse position available within the District Nursing North Sedgemoor team in Sedgemoor Neighbourhood covering Highbridge, Burnham, Cheddar, Axbridge and all the villages in between. Should you be successful the team cross-cover the whole area.
The shift patterns are 8am-4pm, 9am-5pm 10am-6pm or 2pm-10pm. You will also work one in four weekends which include additional unsocial hours enhanced payments. We offer flexible working solutions where possible to ensure a work-life balance and part-time hours will be considered.
You'll need to be a car driver with your own vehicle to visit patients in their own homes. We offer a generous mileage allowance to cover the use of your car for NHS business, which includes both mileage and depreciation.

As a community nurse, you'll provide care to patients in their homes, managing a range of complex health issues and collaborating with various other disciplines.
You'll also work closely with the multidisciplinary team, focused on preventing acute admission and facilitating early discharges. You'll develop a broad range of nursing skills, including IV medication administration, palliative and end-of-life care, complex wound management, catheter and continence care, diabetes management, PICC line care, medication administration, and drain tube management.

Community Nurses work as part of a team providing high quality and compassionate care to individuals. The Community Nurse is an autonomous practitioner who is responsible for the holistic assessment of both complex and simple care needs, the development, implementation and evaluation of care and delegation of care where appropriate to other members of the team. They will support the multidisciplinary team in the delivery of high-quality care to enable individuals to achieve their person-centred goals.
The Community Nurse will take responsibility of the patients care pathway and non-clinical duties in the absence of the Team Leader. They will have the understanding to address complex problems in their day-to-day work, reporting concerns to a Team Leader.
The Community Nurse will be able to work effectively as part of a team and will always act within the limits of their competence, knowing who to ask for help and support as required. Community Nurses will operate within agreed ways of working, following the relevant standards, policies and protocols used in their workplace including the standards in the NMC Code, Professional standards of practice and behaviour for nurses and midwives.
