Theservice is responsible for delivering a wide range nursing care to houseboundpatients, which includes wound management, chronic disease management,promotion of self-care, terminal/palliative care & the administration of IVanti-biotics for admission avoidance.

Asa Neighbourhood Staff Nurse you will support the District Nurse in carrying outnursing care for patients primarily within their home but you will alsoparticipate in clinic sessions.
Enthusiasmand the ability to use your own initiative are essential, along with goodwritten and verbal communication skills and the ability to manage your timeeffectively.
Youwill need the ability to work both as part of a team and as a lone worker,along with the use of a vehicle to undertake any necessary travel in connectionwith the duties of the post.
Youwill be allocated to a specific Neighbourhood; however, it should be noted thatyou are employed to work across the service and may be required to move basesif/when required.
Youwill be required to work flexibly as the service operates 8am 8pm.

JOB SUMMARY

Adapt and provide a wide range of nursing care in home and community based settings
Be accountable for assessing, planning, implementing and evaluating care for patients, reporting and escalating relevant information to appropriate health and social care professionals.
Work in in an integrated and partnership way with primary, secondary, social care, the independent and voluntary sector and others to improve the health and care of individuals, families and communities, particularly the most vulnerable.
Undertake and maintain the required statutory, mandatory and core training and preparation to meet the specific clinical needs of the service.
