Community Specialist Practitioner - District Nurse

Are you a qualified District Nurse with the Specialist Practitioner Qualification? Are you looking for an exciting and challenging role in a friendly and supportive community care team? If so, we would love to hear from you.

We are seeking enthusiastic and passionate District Nurses with excellent communication and leadership skills to motivate and support others in delivering high-quality, proactive, and safe care.

Key Responsibilities:
  1. Lead and manage a team providing community nursing care for a defined locality.
  2. Establish high-quality, evidence-based care programs for referred patients.
  3. Implement and evaluate care plans in collaboration with other agencies, including statutory and voluntary services.
  4. Line manage a team with varying skill mixes.
