Job summary

Meanwood Neighbourhood Care Management in Leeds is seeking a compassionate Social Worker to join our team on a part-time basis. You will assess clients, develop care plans, and coordinate with healthcare professionals to deliver personalized community care.

The ideal candidate will hold a recognised Social Work degree and be registered with Social Work England, with experience in community settings and strong communication skills.

Qualifications

• A recognised degree in Social Work and current registration with Social Work England.
• Proven experience in a social work setting, preferably within community care or similar environments.
• Strong interpersonal and communication skills, with the ability to build rapport with clients and families.
• Excellent problem-solving abilities and a compassionate approach to client care.
• Knowledge of relevant legislation, policies, and best practices in social work.

Responsibilities

• Conducting assessments to identify the needs of clients and develop appropriate care plans.
• Providing ongoing support and advocacy for clients, ensuring they have access to essential services.
• Collaborating with healthcare professionals, community organizations, and families to facilitate comprehensive care.
• Monitoring progress and adjusting care plans as necessary to achieve desired outcomes.
• Maintaining accurate records and documentation in compliance with regulatory standards.
