A health organization in Pembroke Dock is seeking two Band 6 District Nursing Sister/Charge Nurses. Candidates must have community nursing experience and relevant qualifications. This full-time role requires the ability to provide high standards of care, manage teams, and support patient discharge planning. Fluency in Welsh is desirable, and bilingual candidates are encouraged to apply.
An exciting opportunity has arisen for two permanent Band 6 District Nursing Sister/Charge Nurses to join the Pembrokeshire District Nursing Teams based in either Pembroke Dock or Manchester Square.Applicants must have previous community experience and must have already undertaken the Specialist Practitioner Qualification in District Nursing (SPQ) or be willing to work towards. This is a rewarding role requiring the successful applicant to combine excellent clinical practice with day-to-day shift co-ordination, in addition to undertaking leadership and management responsibilities in support of the DNTL. You will be required to formally deputise in the absence or the Senior Sister/ DNTL. This is a permanent full-time position working 37.5 hours per week as 5 days over a 7-day period including weekends and bank holidays. The post holder will need the ability to travel between sites in a timely manner.
Post holder will be required to assess care needs and develop, implement and evaluate programmes of care without supervision. Ensuring high standards of care are delivered to patients and their families/carers within the home/clinic environment. Delegate the delivery of care where appropriate, in line with NHS Wales delegation framework. Supervise the delivery of care and staff involved with its provision. Ensure that the patient's physical, psychological, social and spiritual and fundamental aspects of care needs are met. Be accessible in the community setting to the nursing and wider multi-disciplinary team, patients and service users within primary and secondary care. Continually support and work in conjunction with the Senior Sister and formally deputise in their absence. Deal with complex clinical and managerial situations that arise, critically analysing, seeking appropriate help and making decisions in relation to patient risk and care. Seek appropriate advice/alert agencies when there are any concerns regarding potential/actual risk to any patient or their dependents e.g. child safeguarding, domestic violence, protection of the vulnerable adult, mental capacity and deprivation of liberty.

Ensure that safe and effective discharge planning forms a core component of care planning and delivery from the time of initial assessment, taking into consideration the contributory factors influencing a safe discharge including referrals to primary care and supportive services.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
