Community Response Hub Clinical Team Leader

Are you a clinician with person centred leadership skills with a keen interest in community care? Would you like to be part of the leadership team working in partnership with colleagues and key stakeholders to continue the development of our Community Response Hubs ? This is an opportunity to lead in one of our 3 Community Response Hubs.

We are looking for an enthusiastic and motivated registered nurse/therapist who is committed to working with our local populations to enable people to return home from hospital at the earliest opportunity. The successful applicants will have a proven ability to lead in implementing and sustain changes and ability to work collaboratively with other organisations and bring people together. You will provide via yourself and the team a high quality, effective service to patients throughout their discharge journey from hospital and into their homes.

You will be responsible for all operational management across the team, provide leadership to colleagues from various organisations working together. Provide day to day management of staff within your team including appraisals, supervision, absence management, budget management and rostering. To provide clinical oversight of a complex and varied community caseload.

You will play an integral role in developing the service and aligning services to work together in a home first approach.

To clinically lead, operationally manage and develop an efficient and effective community response hub delivering multidisciplinary health and social care for adults, older people and their carers. Ensuring that the Trust values, policies, procedures and good practice are adhered to and which ensures that adults and older people receive seamless, evidenced based services.

Cheshire and Wirral Partnership (CWP) provides health and care services for a population of over one million people, including mental health, learning disability, community physical health and all-age disability care, as well as the provision of three GP surgeries in Cheshire.

We employ around 4,500 staff across 73 sites and have services across Wirral and Cheshire, as well as Trafford, Warrington, Bolton, Halton and Liverpool. We also provide specialist services for the North West as a whole.
