Job summary

Choice Care Group is seeking a dedicated Outreach Rehabilitation Support Worker to operate in the Sheffield area. The role involves helping individuals regain skills and improve quality of life after injuries, through engaging activities and collaborative problem-solving with neurotherapists and the care team.

Based in the community and in clients' own homes, this role requires a UK driving licence and access to a vehicle.

Qualifications

• Requires UK driving licence and access to a vehicle.
• Previous care or community support experience preferred but not required.

Responsibilities

• Work with neurotherapists to help people regain skills and quality of life after injury.
• Deliver activities and collaborate with the team to find creative solutions.
• Provide community-based support in individuals' homes, communities and workplaces.
• Be flexible with a varied rota, including weekend work; full-time commitment.

Salary: £13.45 per Hour

As this role is in the community supporting individuals in theo own homes, communities and workplaces, it is essential to be a driver and car owner. This role includes working on a varied rota including weekend work (If preferred, there may be the opportunity for you to do some work in our residential rehab service)
