Dorset HealthCare is looking for Community Rehabilitation Assistants to join the Poole Intermediate Care Service. The role involves working with a multi-disciplinary team to provide care in patients' homes, thereby preventing hospital admissions and facilitating discharges. Applicants must have basic IT skills, with NVQ Level 2/3 advantageous. Working conditions involve shifts throughout the year, including weekends.
Exciting opportunities have arisen for new Community Rehabilitation Assistants to join Poole Intermediate Care Service.
The chosen candidates will work as part of a Multi-disciplinary Team caring for patients with an illness in their own homes to prevent hospital admissions and facilitate complex discharges from hospital. You will work alongside qualified Occupational Therapists, Physiotherapists, Nurses, Therapy Technicians, and wider locality teams. Good communication skills and a caring attitude are essential.
Shift working is required as the service operates 365 days a year from 08:00 to 20:00. The successful candidates will need to work some weekends. Full or part-time permanent and temporary hours are available. You must use your car to visit patients at home; car owner/driver is essential, subject to the provisions of the Equality Act 2010.

This position requires an Enhanced DBS check, which the Trust will cover. Applicants who subscribe to the DBS update service can present a valid DBS certificate. Applications requiring current Skilled Worker sponsorship are ineligible.
