Are you a caring individual looking to work in an outstanding rated Trust? A new opportunity has arisen for a Band 3 Community Healthcare Assistant Role within the Frailty hospital at home and older persons services.

Under the supervision and support from the team you will be required to visit patients in their own homes providing clinical care. You will have an active role within the multidisciplinary team in helping to prevent hospital admissions and support patients to remain at home safely.

This is a full-time post, but we would be flexible in considering part-time hours of no less than 22.5hrs for the right candidate.

Please note visa sponsorship for this role is unfortunately not available. Please ensure you hold valid right to work in the UK and will not require sponsorship if successful.

We are seeking a highly motivated, enthusiastic individual with good communication skills and a passion for caring for patients in their own homes.

Experience of clinical work in the community is desirable but training will be offered to the successful applicant.

There is an expectation that the successful candidate will undertake our in-house development program for extended skills which will involve clinical observations, phlebotomy and ECG recording.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service can present a valid DBS certificate instead of requiring a new check.

Access to a car with a valid driving license is essential for this role. Subject to the provisions of the Equality Act, able to travel using own vehicle on Trust business
