Job description

Join our Reablement Team as a Community Reablement Assistant (Level 2) and support adults over 18 in Hampshire needing extra care. Our goal is to maximise their independence and delay or avoid ongoing care, helping them stay at home. Our team, including Community Reablement Assistants, Occupational Therapists, and Sensory Officers, collaborate with health colleagues to support those with short or long-term conditions and those transitioning from hospital.

What you'll do:
  1. Provide Person-Centred Care: Conduct initial home visits and independently perform reablement interventions to restore independence, including intimate personal care, continence care, and moving and handling.
  2. Deliver Outcomes: Perform risk assessments, set goals, update reablement plans for Level 1 Assistants, and monitor service user progress.
  3. Support Operational Delivery: Supervise and coach Community Reablement Assistants and assist Team Leaders, Occupational Therapists, and Sensory Officers with activities.
Knowledge, Experience, and Values:
  • Understanding of the needs of service users, particularly in relation to reablement programmes.
  • Experience in a social or healthcare setting.
  • Demonstrates patience, compassion, empathy, and excellence, treating all service users with dignity and respect.
  • Builds successful working relationships through strong interpersonal and communication skills, and seeks advice from professionals when needed.
Skills and Requirements:
  • Proficient in MS Office (Word, Outlook, Excel).
  • Current driver’s license and access to a reliable vehicle, as travel within Hampshire is required.
  • Understanding of Disability Confident principles.

Working Patterns:

Our service operates from 07:00 to 22:00, 7 days a week. You will work evenings, weekends, and public holidays with enhancements paid according to policy. Please note, sponsorship is not available; applicants must have the right to work in the UK.
