Are you a registered PT/OT/Paramedic or Nurse?

Our innovative Rapid Response & Intermediate Care service is expanding across Buckinghamshire – and we need you to join our Urgent Community Response North and/or South Squad.

  • Your mission: To provide triage expert clinical interventions and rehabilitation as part of a dynamic, tight-knit squad that reacts quickly when people are in crisis.
  • Your aim: To help patients stay in their own home, empowering recovery and preventing unnecessary hospitalisation. You’ll also support early discharge from hospital and patients at the end of life.
  • Your role: Add your expertise to our multi-disciplinary community team working with Physiotherapists, Occupational Therapists, Nurses, Specialist Paramedics, Paramedics, Pharmacists, Community Healthcare Assistant Practitioners or Healthcare Assistants.
  • Who are we? We’re part of Buckinghamshire Healthcare NHS Trust, based in William Rathbone House, Aylesbury (North Squad) and Chichester House, High Wycombe (South Squad) covering 7 locations across the county. Together we operate as one mission force, collaborating with colleagues across all sectors of health and social care.

Main duties of the job

  • Providing triage, comprehensive specialist assessment, review and evaluation of the needs of patients and carers.
  • Being responsible for undertaking an urgent community response to people in their own homes to prevent hospital conveyances and keeping patient safe at home with the appropriate clinical interventions.
  • Allocating appropriate workloads to members of the UCR Squad.
  • Working with a range of professionals to ensure care is appropriate and delivered in a timely manner.
  • Demonstrating professional skills and competence in leadership, care planning / co-ordination, and care and treatment delivery. Support the development of clinical pathways.

Person specification

Skills, abilities & knowledge

  • Clinical assessment, planning, delivery, and evaluation skills specific to your profession.
  • Ability to motivate staff.
  • Excellent communication skills.
  • Knowledge of legal requirements for working with vulnerable adults and those lacking capacity.
  • Ability to develop and maintain partnerships.
  • Managing complex cases.
  • Report writing skills.
  • Teaching patients, carers, and colleagues.
  • IT skills.
  • Knowledge of NHS and social care modernization agendas.

Experience

  • Post or pre-registration placements in relevant clinical areas for therapists: Elderly Care, Orthopaedics, Neurology, Respiratory, Palliative Care, Long-term Conditions.
  • Experience working within multi-agency/multi-disciplinary teams.
  • In-depth experience of community/in-patient settings for at least 2 years post-registration.
  • Experience with electronic clinical systems.
  • Participation in research or audit projects.
  • Experience in staff management, e.g., case allocation to Healthcare Assistants.
  • Experience with the single assessment process.

Special circumstances

  • Valid driving license and ability to travel across Buckinghamshire.

PPE requirements: Buckinghamshire Healthcare NHS Trust requires all colleagues to wear appropriate PPE in accordance with infection prevention and control procedures.
