A healthcare service in the UK is seeking an experienced Mental Health Practitioner to join their team. The role involves working with patients and families, supporting triage and personalised care planning, and promoting recovery. Ideal candidates will have experience in mental health interventions and strong communication skills. Opportunities for training and development will be provided.
This is an exciting opportunity for an experienced Mental Health Practitioner to join the Ashfield LMHT as part of the community transformation programme, in line with the longer-term plan for community services. The transformation includes opportunities to develop skills within SMI pathways (mood affective, complex psychosis, community rehabilitation and non-psychosis including the Cluster 8 and Trauma Pathway). The post holder will be expected to work with patients and their families to support triage of referrals, assessment, personalised care planning and in the provision of specialist interventions. You will promote the recovery of patients and through working with partner agencies and primary care to ensure timely and safe discharge from secondary care.
We are particularly interested in hearing from people with skills and experience working with adults with a range of mental health difficulties who require complex case management/clinical interventions. You will have opportunities to access training and develop your existing skills in all relevant LMHT Pathways. Our team is supportive and experienced and operates within a recovery and psychologically informed model.
