Overview

This is an exciting opportunity for an experienced Mental Health Practitioner to join the Ashfield LMHT as part of the community transformation programme, in line with the longer-term plan for community services. The transformation includes opportunities to develop skills within SMI pathways (mood affective, complex psychosis, community rehabilitation and non-psychosis including the Cluster 8 and Trauma Pathway). The post holder will be expected to work with patients and their families to support triage of referrals, assessment, personalised care planning and in the provision of specialist interventions. You will promote the recovery of patients and through working with partner agencies and primary care to ensure timely and safe discharge from secondary care.

Responsibilities

• Work with patients and their families to support triage of referrals, assessment, personalised care planning and the provision of specialist interventions.
• Promote recovery and coordinate with partner agencies and primary care to ensure timely and safe discharge from secondary care.
• Develop specialist skills across all LMHT pathways within the team and provide leadership in those areas.
• Support students and junior staff through clinical supervision and mentorship, and line manage some staff as required.
• Work within a dynamic multi-disciplinary health team with excellent communication and interpersonal skills; manage complex cases in partnership with other agencies.
