Job description

A Vacancy at Nottinghamshire Healthcare NHS Foundation Trust.

Position: Mental Health Practitioner (Fixed Term)

This is an exciting opportunity for an experienced Mental Health Practitioner to join Newark & Sherwood LMHT as part of the community transformation programme, aligned with the long-term plan for community services. The transformation includes opportunities to develop skills within SMI pathways (mood affective, complex psychosis, community rehabilitation & non-psychosis, including the Cluster 8 & Trauma Pathway).

We are particularly interested in candidates with skills and experience working with adults with a range of mental health difficulties requiring complex case management and clinical interventions.

This post is a fixed-term contract for maternity leave cover, ending in March 2026.

Applicants with no prior NHS experience will typically be placed at the bottom of the band, in line with Agenda for Change. Note that the salary is below the minimum required for sponsorship for a Skilled Worker / Health & Care visa. Therefore, the Trust cannot sponsor for these visas. Key Responsibilities:
  1. Work with patients and their families to support referral triage, assessment, care planning, and provision of specialist interventions.
  2. Promote patient recovery and work with partner agencies and primary care to ensure timely and safe discharge from secondary care.
  3. Develop specialist skills across all pathways within the team.
  4. Support students and junior staff through supervision and mentorship, with potential line management responsibilities.
  5. Manage complex cases in partnership with other agencies, demonstrating excellent communication and interpersonal skills.
Team and Organizational Overview:

#TeamNottsHC comprises over 11,000 colleagues delivering services across Nottinghamshire and surrounding regions, including community, mental health, forensic, and offender healthcare services from over 200 sites. The Trust is one of the largest in the East Midlands, hosting national and regional services, and promoting diversity and wellbeing through various staff networks and support services.

Day-to-Day Duties:
  1. Caseload management and acting as care coordinator for community patients.
  2. Referral triage, initial assessments, and one-to-one interventions.
  3. Conduct and document comprehensive need/risk assessments and develop treatment plans.
  4. Apply specialist skills to maintain professional competence.
  5. Maintain communication networks with clients, carers, and other health/social care professionals.
  6. Use interpersonal skills to manage emotive situations during assessments and planning.

A full UK driving license and vehicle for business use are required; reasonable adjustments will be made for disabled applicants in line with the Equality Act 2010.

Applicants will need to pay for their DBS check, deducted from salary over the first two months. The DBS application cost is £26.40 (standard) or £54.40 (enhanced). Enrolment in the DBS Update Service is encouraged, with an annual fee of £16.
