We are developing an Integrated Community model, focusing on new ways of working within integrated mental health teams to support people living in their communities with mental health challenges.
Main duties of the job
You will be a Registered Mental Health Nurse, preferably with adult and community mental health experience working with severe and enduring mental health service users. You will be responsible for care coordinating service users with complex long-term mental health problems who find it difficult to engage. You will have experience in assessing and formulating individual intervention plans and outcome measures for adults with complex mental health problems, with current knowledge of NHS policies and practices in mental health. You will participate in and support the CMHT duty system and be responsible for managing the mental health needs of service users referred to the team.
The successful applicants will also have previous experience of the Recovery Model, a good working knowledge of psychotropic medication and its effectiveness, and the ability to deliver a range of therapeutic interventions. Additionally, you will demonstrate the ability to undertake and manage clinical risks and develop robust clinical management plans for service users within the service. You will support the team manager and clinical lead to adhere to and deliver high-quality service and have in-depth knowledge of national quality indicators.
