Community Psychiatric Nurse (CPN) – NHS Agency Contract | Wisbech, Cambridge

Service Care Solutionsis currently recruiting experiencedCommunity Psychiatric Nurses (CPNs)for a full-time agency contract within theNHS Adults Community Mental Health Teambased intheWisbecharea.

Contract Details:

• Location:Wisbech, Cambridge (Community-based role)
• Hours:Monday to Friday | 9:00 AM – 5:00 PM
• Pay Rates:
• £27.00 per hour (Umbrella)
• £21.71 per hour (PAYE, inclusive)

Role Overview: The successful candidate will manage a caseload of up to 25 patients withsevere and enduring mental health conditions. This is a community-based role involving:

• Administeringdepot injectionsand managing patients onClozapine
• Participating in theduty rotaandassessment rota
• Completing all clinical documentation includingrisk assessments,care plans, andcrisis plans
• Managingsafeguarding concernsand liaising with appropriate agencies
• Working bothautonomouslyand as part of amulti-disciplinary team
• Delivering meaningfulinterventionsto support recovery and continuity of care
• Engaging with external services to ensure a cohesive care journey for each patient

Key Responsibilities:

• Manage a defined caseload under theCare Programme Approach (CPA)
• Conduct comprehensivemental health and risk assessments
• Develop and implementindividualised care plansandtherapeutic interventions
• Providecrisis interventionand ongoing support
• Administerdepot medicationand monitor treatment compliance
• Collaborate within amulti-disciplinary team (MDT)
• Liaise with GPs, social workers, and other healthcare professionals
• Maintain accurate and timelyclinical recordsin line with NHS standards
• Supervisejunior staffandstudent nursesas appropriate
